Hourly Pay Range $56.44 - $70.58. The hourly pay rate offered is determined by the candidate's expertise and years of experience, among other factors.

  Advanced Practice Provider (APN/PA) – Concierge Medicine - Internal Medicine
• Location:
  
  915 S. Waukegan Road, Lake Forest, IL 60045
• Full Time/Part Time:
  Full time, 40 hours/week
• Hours:
  
  Monday - Friday between 7am - 6pm
• Saturday rotation (≈ once a month) 8am - 12pm
• Call rotation
• Outpatient - Onsite and telehealth

Exciting opportunity to join Endeavor Health in the Primary Care Department. Under the direction of collaborating physicians, provide care to patients by obtaining detailed history, performing physical examinations, ordering imaging, laboratory tests, and diagnostic procedures, and interpreting results. Instruct patients on therapeutic regimens and pre/postoperative instructions. Maintain records and write progress notes on status, treatments, and procedures. Follow patients through their course of treatment in the clinic and hospital setting.

Collaborate with physicians and the health care team to achieve optimal patient outcomes. Prescribe pharmacologic and non-pharmacologic therapies as appropriate. Demonstrate evidence-based practice. Participate in development, implementation, and evaluation of standards of practice. Assess learning needs of patients and families across the continuum. Employ strategies, interventions, and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnoses, and prescribe treatments under protocols in direct management of acute and chronic illness and disease.

Promote wellness and prevent illness and injury in various settings for individuals, families, and communities. Work autonomously and in interdisciplinary teams as resources and consultants. Contribute to safety outcomes and promote high quality patient experience. Participate in development of service line goals and support the strategic plan of the organization. Adhere to all Endeavor Health policies and procedures.

• License:
  Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required;
  Federal DEA and Illinois Controlled Substance License required
• Education:
  
  Completion of master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience:
  
  Minimum of three years primary care/internal medicine APN/PA experience preferred

• Minor procedures such as skin tag removal
• Gynecological examinations (Pap smears, breast exams, pelvic exams)
• Diagnostic tests / EKG
• POCT such as COVID/FLU/Strep
