We are looking for Qualified Licensed Practical Nurses

Requirements to Register as an Independently contracted Caregiver:

  • Valid Driver's License or Photo ID
  • Social Security Card
  • LPN license
  • Level 2 background screening with ACHA fingerprints
  • Physical within (6) months that states you are FREE OF ALL COMMUNICABLE DISEASES (cannot be signed by a Chiropractor)
  • CPR (American Heart Association, American Red Cross, or American Safety & Health Institute)
  • HIV/AIDS certificate (minimum 1-hour)
  • 30 hrs of IV infusion training (Optional)
  • Auto registration and auto insurance if providing transportation
  • Professional Liability Insurance (suggested)
  • 2 Work Verifications AND 2 professional and/or personal references
