This full-time position supports patients in Leawood, KS and surrounding areas and offers a $10,000 first-year success bonus for key achievements.

• Lead and collaborate in the management and delivery of patient care plans.
• Support patients and families across Leawood, KS and surrounding areas.
• Maintain critical deadlines for charting and documentation.
• Utilize tablets, mobile phones, and EMR software to manage patient records.

• RN license in the state of employment.
• Graduate from an approved school of practical nursing.
• Minimum one year of home health experience as an RN and clinical competence to deliver quality patient care.
• Valid driver’s license with ability to spend approximately 20% of the day driving to patient locations.
• Ability to self-manage time and schedule.
• Demonstrated experience with tablets, mobile phones, and EMR software.
