Position: Staff LPN-Functional Care

Summary

At the direction of the Registered Nurse (RN) and/or Licensed Practitioners, the Staff Licensed Practical Nurse (LPN) contributes to meeting patients’ priority needs by assessing and participating in interdisciplinary care planning, providing delegated care, and facilitating timely communication of the patient’s response. The LPN also supports an efficient and effective work environment through professional communication and documentation.

Principal Job Functions
  • Commits to the mission, vision, beliefs and consistently demonstrates our core values.
  • Contributes to the assessment and reassessment based on the patient’s presenting problems/needs in accordance with Bryan Medical Center’s procedures.
  • Communicates pertinent initial and ongoing patient data to the RN for identification of nursing priorities of care and desired outcomes/goals.
  • Utilizes appropriate standards of care based on the patient’s diagnosis and age‑specific needs.
  • Executes therapeutic and/or supportive interventions under the direction of the RN.
  • Provides education for the patient and family to meet identified patient needs according to their needs, abilities, preferences and readiness to learn.
  • Participates in the evaluation of the patient’s response to and effectiveness of interventions.
  • Incorporates cultural considerations into the plan/delivery of care as directed by the RN.
  • Includes family or significant others in the care of the patient as appropriate.
  • Documents patient care and related information according to Bryan Medical Center procedures for documentation.
  • Enters patient care orders into the computer accurately and consistently according to policy and procedure.
  • Provides care in conjunction with other members of the interdisciplinary team.
  • Provides close observation or one‑to‑one care, as directed by the RN, to prevent patients from wandering or potential injury.
  • Contributes to the patient’s transition to the next level of care.
  • Assumes responsibility in a shared or delegated assignment and supports team member’s practices through timely communication.
  • Serves as a role model to colleagues, students and other care providers.
  • Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in the field of expertise.
  • Participates in meetings, committees and department projects as assigned.
  • Performs other related projects and duties as assigned.

Required Knowledge,

Skills and Abilities
  • Maintains clinical competency as required for the unit, including but not limited to age‑specific competencies relative to patient’s growth and developmental needs, annual skill competency verification and mandatory education and competencies.
  • Knowledge of federal and state regulations related to nursing applications and relative practice area.
  • Knowledge of computer hardware equipment and software applications relevant to work functions.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to perform crucial conversations with desired outcomes.
  • Ability to establish and maintain effective working relationships with all levels of personnel and medical staff.
  • Ability to work in a fast‑paced environment related to changing patient needs, including work with patients with acute, chronic, and complex disease processes, and those who are dying.
  • Ability to problem‑solve and engage independent critical thinking skills.
  • Ability to maintain confidentiality relevant to sensitive information.
  • Ability to prioritize work demands and work with minimal supervision.
  • Ability to maintain regular and punctual attendance.
Education and Experience

Graduation from a School of Practical Nursing with current licensure from the State of Nebraska or an approved compact state of residence as defined by the Nebraska Nurse Practice Act is required. Must provide evidence of certification and/or instruction for the practice area as required by State or Federal standards/regulations.

Other Credentials / Certifications

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
