Program Overview:

ACT is an evidence-based practice with widely demonstrated success in helping people with serious mental illness who have not benefited from traditional treatment programs. ACT/AOT works to support individuals living in the community as they work towards their recovery goals. With its multi-disciplinary staff, ACT/FACT provides comprehensive, assertive, community-based services. In order to achieve the outcomes demonstrated by ACT/AOT programs, the teams must follow the principles of the model, including meeting staffing and service delivery expectations, and following key processes. The primary goal of ACT is to encourage and facilitate those with serious and persistent mental illness (SPMI) in their recovery process. Paramount is facilitating the recovery process within the community in which the individual lives. ACT/AOT delivers services that promote the recovery process in the community where the individual lives.

Services Location:

ACT/AOT delivers services in the community where consumers live and other community settings such as: a coffee shop, work place or Merakey office. The ACT/AOT office hours are Monday through Friday 8:00 am until 8:00 pm and Weekend coverage with added 24/7 coverage as needed. The team also has a 24/7 crisis line where staff are available for crisis and emergency support services.

Position Summary:

The Psychiatric Nurse Practitioner is part of a multidisciplinary team that provides comprehensive mental health care to patients living with chronic mental illness. The NP will work in collaboration with our psychiatrist and registered nurses to diagnose and manage patients' mental health concerns. The NP will provide services in the community and will ride along with another staff member to provide these services.

General Duties / Responsibilities:

* The Psychiatric Nurse Practitioner will be responsible for conducting psychiatric patient evaluations to determine medication needs, documenting evaluations and interactions with the patient in the electronic health record.
* The Psychiatric Nurse Practitioner will also be responsible for prescribing pharmacologic therapies including controlled substances, monitoring progress and making necessary changes to patients' medications.
* The Psychiatric Nurse Practitioner will facilitate direct patient admissions into hospitals when needed.
* The Psychiatric Nurse Practitioner will participate in weekly multidisciplinary team meetings to develop coordinated treatment plans.

Schedule:

32 hours per week.

* License to practice in the state of California as a Psychiatric Nurse Practitioner (PMHNP-BC).
* Experience in a community mental health setting is preferred.
* Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment.
* Bilingual a plus.
