Community Nurse Band 5

Community Team - Malmesbury
37.5 Hours/ WeekFixed Term - 6 Months

Are you passionate about nursing and providing health care in people’s own homes?

Do you have the commitment to make a difference to people’s lives?

Are you keen to learn and develop within a rewarding Community Nursing role?

The Malmesbury and Royal Wootton Bassett Community Team is looking for an inspirational Registered Nurse to deliver a high standard of quality care to patients within the community setting. Wiltshire Health and Care is proactive in developing community specific training and can offer a supportive and friendly environment in which to learn new skills.

Community Nurses deliver care in patients’ homes and residential homes as part of a Multi-Disciplinary Team, linking closely with the GP Practices and other community services in the local areas. We work with the aim of preventing unnecessary admissions to hospital and in assisting people to manage their conditions in their own homes. The Malmesbury Community Team is split into three clusters (Malmesbury/Sherston, Purton/Cricklade and Royal Wootton Bassett) and we are a multidisciplinary team including Nurses, Therapists, Clinical Leads, the Home First Team, Care Coordinators and other staff.

The team is based in the Malmesbury Primary Care Centre and operates a shift pattern from 07.00 – 22.00. Nursing staff will be expected to work weekends and will be expected to cover early and late shifts., Evidence of up to date clinical professional development is essential and experience in front line NHS community patient care is preferable.

This is a 6 month fixed term post. Applicants need to have excellent clinical skills, well developed communication skills and be able to work under pressure. The role is demanding but rewarding, and we are looking for an enthusiastic, motivated and hardworking individual who can work flexibly and efficiently. We are a busy, friendly team, who would welcome the opportunity to work with a new member of staff willing to contribute to the on-going development of the services we provide.

For enquiries or to arrange an informal visit please contact the team office on 01666 827567 and speak to Mari-Louise Scott or Samantha Atkin.
