Position: Registered Nurse - Home Health /Hospice {170015}
Registered Nurse – Home Health / Hospice Pay: $47–$51/hour, paid weekly

Location:

Local travel required within assigned service area

Hours:

Flexible scheduling based on patient needs with on-call rotation responsibilities as assigned

Qualifications:

Current state Registered Nurse (RN) license required Current CPR certification required Valid driver’s license required Reliable transportation and current auto insurance required 1–2 years of Registered Nurse experience in a clinical care setting required 2+ years of home health experience required Homecare Homebase software experience required OASIS experience required CHPN certification preferred Strong clinical, communication, and organizational skills Ability to work independently and collaboratively within a team environment Ability to travel locally and maintain a flexible schedule Education Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program required Bachelor of Science in Nursing (BSN) preferred

Key Responsibilities:

Assess home health and/or hospice patients in stable and emergent situations Develop, implement, document, and evaluate individualized Plans of Care (POC) Coordinate patient care with physicians, caregivers, and interdisciplinary team members Maintain accurate and timely clinical documentation and patient records Provide patient and family education regarding medications, treatments, home safety, diet, and self-care Utilize infection control practices in accordance with OSHA standards Assess and manage patient pain while educating patients and families on pain management techniques Supervise and provide clinical direction to support staff including HHA/STNA/LVN team members Perform clinical nursing duties including:
Wound care and treatment IV care and venipuncture Inhalation therapy Phlebotomy Catheter and drain care Bowel and bladder training programs Coordinate referrals to appropriate healthcare agencies and services as needed Participate in interdisciplinary meetings and collaborate with healthcare professionals to ensure quality patient care Monitor assigned cases for compliance with payer and regulatory requirements Perform additional duties as assigned
