Overview
All Care is seeking a VNA Nurse to provide skilled nursing care to patients in their homes and coordinate all aspects of care to achieve optimal clinical outcomes. This role develops and manages individualized care plans, collaborates with interdisciplinary team members and caregivers, and ensures compliance with agency, regulatory, and professional standards.

Position Type:

- Practice independently in the community as part of an interdisciplinary care team.
- Conduct patient assessments and develop individualized, goal-oriented Plans of Care based on best practices.
- Manage an assigned caseload in accordance with agency productivity and quality standards.
- Coordinate patient care with physicians, caregivers, and interdisciplinary team members to ensure continuity of care.
- Complete timely and accurate clinical documentation, including visit notes, care coordination, telephone communications, and physician orders.
- Perform and document formal and informal case conferences.
- Supervise and coordinate services provided by discipline extenders (LPNs, PTAs, COTAs) and home health aides.
- Monitor patient progress and adjust care plans as needed to support positive clinical outcomes.
- Promote improvements in key quality measures, including hospitalization reduction, mobility, wound healing, medication management, pain control, and respiratory status.
- Valid driver’s license is required.
- Active Massachusetts Registered Nurse (RN) license is required.
- Minimum of one year of clinical nursing experience under qualified supervision.
- Home health or community health nursing experience preferred.
- Excellent communication and interpersonal skills to effectively educate and support patients and their families.
- Ability to work independently and manage patient care in a home setting.
- Proficiency in using electronic health record (EHR) systems for documentation and communication.
- Problem-solving skills to modify treatment plans based on patient progress and challenges.
