Job SummaryThe Center for Living & Rehabilitation is seeking a dedicated and compassionate Registered Nurse (RN) to join our team! As a key member of our healthcare staff, you will play a vital role in providing high-quality patient care to our residents.
Responsibilities- Provide skilled nursing care to residents in our Subacute Rehab wing
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor and report changes in resident condition to the nursing team and medical staff
- Communicate effectively with residents, families, and staff to ensure seamless care transitions
- Participate in quality improvement initiatives to enhance the resident experience
- Maintain accurate and detailed records of resident care
- Current RN licensure in the state of Vermont
- Ability to work effectively as a team member in a fast-paced environment
- Strong communication and interpersonal skills
- 10k sign on bonus
