The Registered Nurse (RN) provides and directs the provision of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The RN coordinates total plans of care with other health care professionals, and helps to achieve and maintain continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, family, and community resources.
Responsibilities- Make the initial and/or comprehensive nursing evaluation visit, determine the primary focus of care, develop the plan of care within state-specific guidelines, and submit accurate, complete, and timely documentation per policy.
- Make home visits to clients in designated geographic territories.
- Initiate, develop, implement and make necessary revisions to the plan of care in collaboration with the physician and other health care professionals involved in care.
- Communicate current health status and needs of clients to the appropriate staff.
- Provide on-call coverage / respond to calls.
- Document complete plan of care for errors and findings.
- Complete initial assessments and aide assignment sheets for client personal/attendant care.
- Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer source.
- Audit and review medication logs.
- Current RN licensure in state of practice.
- Current CPR certification required.
- Registered Nurse 3 years (Preferred).
- Current driver license and vehicle insurance.
- Must be 18 years of age or older.
- Must be able to pass a TB skin test & drug screen.
- Must be able to pass a criminal history check.
