Licensed Practical Nurse (LPN) responsible for providing patient care in a fast‑paced outpatient setting, including rooming patients, triage, and clinical support under the direction of a provider. Ensures efficient patient flow while delivering high‑quality, patient‑centered care.

• Room patients and maintain efficient patient flow for scheduled and walk‑in visits
• Perform triage (in‑person and phone) and prioritize patient needs per protocol
• Obtain and document vital signs, medical history, and clinical information in the EMR
• Communicate patient concerns and clinical findings to providers
• Assist providers with exams, procedures, and patient care
• Administer medications, immunizations, and treatments as ordered
• Process prescription refills and patient calls/messages
• Perform or assist with in‑office testing and specimen collection
• Provide patient education and support for care plans and follow‑up
• Maintain accurate, timely documentation and uphold confidentiality standards

• Active, unrestricted LPN license in Vermont
• Current CPR certification
• Strong clinical assessment and triage skills
• Excellent communication and interpersonal skills
• Ability to multitask and thrive in a fast‑paced environment

This is a bargaining union position. University of Vermont Health - Porter Medical Center is located in the heart of beautiful Middlebury, Vermont. We are comprised of a 25‑bed acute care facility, a 98‑bed skilled nursing facility which includes a memory care unit, short‑term rehabilitation unit and a long‑term care unit, and a network of primary care and specialty medical practices throughout Addison County and the town of Brandon.
