Summary

Midland location
Afternoon shift: 12p - 9p
Staff Registered Nurse provides professional and comprehensive nursing care within the scope of agency services for patients requiring skilled clinical home care services. Activities are patient/client focused and utilize the nursing process of assessment, planning, intervention, and evaluation in accordance with nursing practice standards, physician orders, and agency policies.

May be eligible for sign on bonus up to $7,500.

Responsibilities
  • Regularly assess patients' needs and provide skilled nursing services as prescribed by the physician authorized plan of care (20%).
  • Initiate and revise plan of care for assigned patients (20%).
  • Provide nursing care or arrange for services through specialized staff during ongoing visits to a group of patients, sharing the case load (20%).
  • Coordinate patient care by evaluating outcomes and providing ongoing feedback to others involved (15%).
  • Coordinate care provided by Licensed Practical Nurses (LPN) and Home Health Aides and supervise LPN as needed (15%).
  • Notify physician of changes in condition and obtain new orders (10%).
Other Duties and Responsibilities
  • Function as case manager for assigned case load if assigned.
  • Implement preventative and rehabilitative nursing procedures and utilize community and agency resources.
  • Utilize Agency documentation system to record services.
  • Assist in maintenance of patient records.
  • Provide information for informed decision-making to patients, caregivers, physicians.
  • Establish Home Health Aide care plan and supervise according to guidelines.
  • Make joint visits with other disciplines to assure consistency of care.
  • Function as part of multidisciplinary team in coordinating care including planning for discharge.
  • Act as a preceptor and participate in orientation of new staff.
  • Participate in agency, community, home health organization activities.
  • Participate in staff meetings, in-service education programs.
  • Comply with documentation requirements for personnel and activity records.
  • Carry out work according to agency policies and procedures, reflecting standards for timeliness, accuracy, productivity.
  • Maintain confidentiality of patient and agency information.
  • Adhere to agency standards related to personal appearance and behavior.
  • Participate in weekend, holiday rotations and on-call duties.
  • Participate in agency performance improvement process.
  • Assist in agency community education activities.
  • Utilize Microsoft® Windows; may participate in further learning opportunities.
In Summary

  • Participate in organization-sponsored training.
  • Other duties as assigned.
Certifications and Licensures
  • Basic Life Support (BLS) – within 1 month of hire or transfer.
Qualifications
  • Driver’s License.
  • Vehicle Insurance.
  • Registered Nurse (RN).
  • Fingerprinting.
  • Team Advanced:
    Techniques for Effective Aggression Management – within 6 months of hire or transfer (employees on or before 12/31/2023 have 24 months to complete certification).
Required Education

Associate Degree.

Experience, Training and Skills
  • Able to function as part of a team.
  • One year of direct patient care experience is preferred.
  • Successful completion of agency orientation and competency programs.
  • Excellent verbal and written communication in English.
  • Adaptability to frequent changes.
  • Recognize needs and concerns of people; maintain constructive interpersonal relationships.
  • Work independently, demonstrating problem solving and organizational skills.
  • Ability to drive under hazardous road conditions.
  • Work where employee may be exposed to communicable diseases in unpredictable home situations.
Physical/Mental Requirements and Typical Working Conditions
  • Exposure to stressful situations, trauma, grief and death.
  • Wear personal protective equipment including latex materials.
  • Move freely about the facility with or without an assisted device.
  • Overall vision and hearing necessary; may use devices.
  • Frequently sit, stand, walk for long periods; may require postural changes.
  • Exposure to blood borne pathogens; must follow blood borne pathogen and TB testing.
  • Handle multiple tasks, work independently; regular attendance; be awake.
  • Handle frequent hand movements; may repeat movements regularly.
  • Physical demand: handle lifting/carrying 10-50 lbs as specified.
