Location
  • Tri Health Heart and Vascular Care Montgomery – Primarily at 10506A Montgomery Rd. Montgomery, OH 45242
  • Working at Wilmington location (when someone is sick/PTO).
Work Schedule
  • Part-Time (40 hours bi-weekly)
  • Day shift - 8:00 AM– 4:30 PM
  • No weekend or holiday rotation

Job Requirements
  • Associate's Degree or Diploma in Nursing
  • For Breast Center positions, Certified Breast Care Nurse (ONCC) is required within one year of hire
  • Basic Life Support for Healthcare Providers (BLS) Registered Nurse
  • Current Ohio licensure
  • Knowledge of the specialty or population served
  • Ability to work effectively on a multi-disciplinary team
  • 1-2 years experience in Nursing
Job Overview

This position provides nursing services, either directly or indirectly, to participants in a specific program, specialty department, or practice. It conducts initial assessments and regular re-assessments, provides ongoing nursing services, works as part of an interdisciplinary team, focuses on prevention, early detection, acute and chronic illness. If an RN from a physician practice works in an inpatient area, the RN will have the capability to write verbal orders, enter orders, and pend access notes, and will be required to attend physician THC training.

Job Responsibilities
  • Plans care for participants and families based on assessment, standards of care, and optimal outcomes. Initiates and individualizes appropriate patient care guidelines, POC or clinical pathways. Maintains current plan of care by updating as needed. Develops both short and long term goals. Participates as a member of the Interdisciplinary Team by identifying caregiver and participant needs, recommending interventions, and collaborating with other health care team members for mutually determined solutions.
  • Performs technical skills according to hospital's and department's policies/procedures, appropriate competencies, and accepted standards of practice. Implements POC. Prioritizes workload according to participants' needs and requests. Bases interventions on clinical data, desired outcomes, the nursing process, and standards of care. Documents interventions and outcomes. Utilizes critical thinking in daily care including accurately receiving, interpreting and executing physician orders.
  • Evaluates the plan of care based on optimal patient outcomes. Documents the participant/family response to care including teaching. Collaborates with other members of the health care team to revise the plan of care as needed. Supervises care delegated to other health care team members.
  • Demonstrates responsibility: identifies areas for self-improvement, functions in relief charge/resource roles as requested, completes assignments within scheduled time frames, assists with student learning experiences, maintains current knowledge in area of practice, and demonstrates knowledge of organizational and unit changes.
  • Conducts and documents comprehensive patient assessments with emphasis on health promotion, prevention, and early detection to plan and implement care directed toward optimal outcomes. Includes health counseling and teaching needs in assessments. Communicates assessment findings to other health care providers as appropriate.
