About the Role Join Bayshore Health Care, a leading provider of home and community health care services, as a Registered Practical Nurse or Licensed Practical Nurse working evening shifts. Your clinical expertise will ensure clients receive high‑quality, personalized nursing care in the comfort of their own homes.
Responsibilities Provide comprehensive nursing care tailored to individual client needs.
Conduct assessments, document findings, and maintain accurate patient records.
Offer compassionate support to improve clients’ physical and emotional well‑being.
Build trusting relationships with patients and families, becoming a trusted partner in health journeys.
Collaborate with interdisciplinary teams and communicate effectively regarding care plans.
Education & Qualifications Graduate from an accredited/recognized School of Nursing.
Current RPN or LPN registration in good standing with the applicable provincial licensing body.
Valid CPR certification.
Valid driver’s license, auto insurance, and reliable vehicle.
Minimum of 2 years nursing experience (community nursing experience preferred).
Excellent assessment, documentation, written, and communication skills.
Requirements for New Hires Clear criminal background check with vulnerable‑sector screening.
Proof of being free from TB.
Two supervisory references.
