As a Nurse Practitioner with the ByWard Family Health Team, you will deliver a full spectrum of healthcare services, emphasizing health promotion and disease prevention. Collaborate with physicians and allied health professionals within our shared care model while integrating evidence-based practices to ensure quality outcomes.
Key Responsibilities:
• Manage care for patients with various disorders
• Evaluate and adjust care plans based on patient outcomes
• Collaborate on developing goals and activities
• Integrate current research into clinical practice
• Maintain a safe environment for all stakeholders
Requirements:
• Current Registered Nurse Extended Class certificate
• Graduate of a recognized Nurse Practitioner program
• Minimum two years of clinical experience as NP
• Bilingualism in French and English preferred
• Excellent assessment and communication skills
