Home Health Case Manager (RN/LPN) – OASIS & 485 Specialist

We are seeking a detail-oriented and experienced Home Health Clinician who thrives on clinical excellence and precise documentation. The ideal candidate will have a deep understanding of the OASIS-E assessment and the development of the CMS-485 Medical Plan of Care. You will be the clinical architect of our patients' home health journeys, ensuring that every assessment reflects the true clinical picture and every plan of care is geared toward optimal outcomes.

• Plan of Care Development:
  Create, coordinate, and update the Form 485 (Plan of Care) for each patient, ensuring all physician orders, frequencies, and goals are accurately captured.
• Clinical Accuracy:
  Ensure all documentation is compliant with Medicare/Medicaid regulations and state licensing standards.
• Care Coordination:
  Collaborate with the clinicians to adjust the Plan of Care as the patient’s condition evolves.
• Quality Review:
  Participate in internal clinical record reviews and quality improvement initiatives.

• Licensure:
  Current, unencumbered RN or LPN license in the State of Indiana.
• Experience:
  
  Minimum of 2+ years of recent home health experience is strictly required.
• Technical Mastery:
  Proven expertise in OASIS-E documentation and a thorough understanding of the 485 process.
• Communication:
  Strong ability to translate complex clinical data into a clear, concise Plan of Care.
• Tech-Savvy:
  Comfortable using Electronic Medical Record (EMR) systems for real-time documentation.
