Location: North Brunswick Township

As a Care Manager, you play a part in addressing the youth mental health crisis by acting in response to their voice!

Skills / Requirements
  • Bachelor's Degree in a Human Service field.
  • At least one year post undergraduate/graduate experience providing family and youth services within a non-profit social services setting.
  • Excellent communication, facilitation, conflict-resolution, assessment and system negotiation skills.
  • Creativity and enthusiasm are essential.
  • Ability to engage youth.
  • Ability to work as a team.
  • Ability to demonstrate professionalism at all times.
  • Must have a valid driver's license and vehicle.
  • Must be available to work in the community and/or the office for an 8-hour shift during core hours of 8am-7pm, 5 days a week Mon-Fri to meet the needs of the families served.
Job Description
  • Provide coordination and/or direct provision of assessment, service planning and care management services to youth and families in a manner that is respectful, dignified and that enhances family self-sufficiency.
  • Assist families in overcoming obstacles by utilizing and building upon their strengths and vision.
  • Play a critical role to ensure the mission is carried out through the work.

Salary Range

$52,500 - $57,500 annually
