Description:

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit.
The individual must demonstrate knowledge of the principles of growth and development across the life span.
The LPN must possess the ability to assess data reflective of the patient's status.
The LPN will interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs.
The LPN must provide care needed by adult and geriatric patient groups.
The LPN provides direct and indirect skilled nursing care for assigned residents.
The LPN follows defined policies and procedures under the supervision of a Registered Nurse.
The LPN maintains a collaborative relationship with other nursing staff, physicians, and ancillary staff.
The LPN is responsible for maintaining an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
The LPN collects data relevant to the resident's physical and psychosocial condition on an ongoing basis.
The LPN communicates findings to the Registered Nurse and other members of the Interdisciplinary Team.
The LPN contributes to the creation of a developmentally age-appropriate and culturally relevant plan of resident care.
The plan reflects nursing diagnosis and interventions to achieve desired resident outcomes.
The LPN involves the resident, family, and healthcare team in the care process while promoting a healthy, safe environment.
The LPN recommends revisions to the plan of care after evaluation of resident outcomes.
The LPN indicates the need for referrals to ensure all aspects of the resident's care have been addressed.
The LPN collaborates with the resident, family, and members of the Interdisciplinary Team, assuring customer satisfaction.
The LPN maintains current knowledge and competency in nursing practice.
The LPN participates in the mentoring and orientation of staff.
The LPN considers factors related to safety, cost, and impact when planning the delivery of care.
