PositionSummary
We are looking for an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to provide primary and palliative care to homebound patients in the Carrollton, Villa Rica, and Newnan, Georgia area on a full‑time basis. You will apply medical techniques and principles under physician supervision, caring for a diverse set of patients with chronic diseases.
Responsibilities- Provide high‑quality care to primary and palliative patients in and around Atlanta County, GA (Newnan County)
- Perform physical examinations, obtain and update medical histories, and develop personalized care plans
- Communicate effectively with office staff and clinical operations across internal platforms
- Provide patient‑centered care to a panel of patients
- Develop impressions, diagnoses, and treatment plans
- Foster trust and consult with patients, families, and caregivers during visits
- Order diagnostic testing as medically necessary
- Prescribe medications, supplies, and appropriate medically necessary interventions
- Board certified with a valid NP or PA license in the state of Georgia
- Strong clinical skills with the ability to accurately and confidently assess, diagnose, and treat patients
- Positive demeanor and strong people skills
- Ability to problem‑solve and operate independently
- 1–2 years of primary‑care experience with geriatric patients (hospice experience highly preferred but not required)
