Shift: Monday - Friday (8am-5pm), with some weekend coverage required

Join our dedicated healthcare team and provide essential care to patients in their homes. As a Home Health/Hospice Registered Nurse, you will have the opportunity to build strong relationships with your patients and their families, ensuring they receive the highest quality of care during their most vulnerable times.
- Conduct comprehensive assessments using OASIS (Outcome and Assessment Information Set)
- Provide skilled nursing care to home health and hospice patients
- Travel to patient homes and deliver personalized care plans
- Manage and adapt to changes in patient schedules and care needs
- Utilize the EMR system Health Wyse (Careficient) for documentation
- Participate in emergency call coverage as needed
- Maintain a valid driver's license and reliable transportation
- Current BLS certification required
- Background check conducted by the facility
