About This Role
The VNA Hospice RN Case Manager provides skilled care and case management to clients and families, ensuring adherence to professional hospice standards and promoting quality of life through direct care, teaching, and counseling.
Highlights- Pay: $37.01 - $53.91/Hourly
- Location: Northampton, MA
- Facility: VNA & Hospice of Cooley Dickinson, Inc.
- Schedule: Full-Time, Day shift
- Provide skilled care and case management to clients/families.
- Communicate and document observations and assessments.
- Promote and maintain quality of life for clients/caregivers.
- Utilize nursing theory and processes in care delivery.
- Graduate of an approved, accredited school of professional nursing required; BSN preferred.
- Minimum of one (1) year medical/surgical nursing experience required; hospice/home care experience preferred.
- Current RN License in Massachusetts.
- Current CPR certification required.
- Current driver’s license with a good driving record required.
