Position: ADMISSION DISCHARGE NURSE (RN/LPN)

Sign on bonus: $5,000

Salary range (RN/LPN): $38.00 - $50.00/hr based on experience & license

This position is Monday through Friday, 11 a.m. – 7 p.m., and includes some on‑call responsibilities.

Pay or shift range
  • Pay or shift range: $38 USD to $50 USD. The estimated range is the budgeted amount for this position. Final offers depend on skill set, experience, location, qualifications and other job‑related reasons.
Primary Purpose

Provide preadmission clinical review of patient referrals and coordinate the discharge process.

Responsibilities
  • Maintains resident census consistent with the admission strategy with Admission Coordinator.
  • Develops good rapport and working relationships with BHCS Liaison; area discharge planners; and appropriate social agencies to promote the facility’s reputation and encourage referrals.
  • Facilitates and/or directs admission process.
  • Advocates and serves as spokesperson for resident concerns.
  • Completes nursing assessment, care plan, and documentation.
  • Provides report to nursing staff.
  • Gathers, documents, and obtains MD order for admission medication list.
  • Completes medication reconciliation.
  • Orders medications from pharmacy.
  • Initiates Medicare forms on admission.
  • Manages consents.
  • Identifies potential room review for infections such as MRSA, VRE, MDRO.
  • Identifies equipment needs.
  • Identifies clinical issues.
  • Notifies Admissions of decision.
  • Greet residents to appropriate rooms.
  • Performs all assessments for admission process and writes nurse’s note.
  • Develops care plans based on assessments and physician orders.
  • Creates C.N.A. Care Card.
  • Reports resident status to nurses and C.N.A’s.
  • Facilitates discharge planning and discharge process.
  • Writes referral to transferring agencies.
  • Assures patient has a 7‑day supply of medications.
  • Observes that discharge order is obtained and written.
  • Reviews discharge paperwork and discharge note.
Qualifications
  • Massachusetts RN or LPN license.
  • Minimum of 2 years clinical nursing experience.
  • Ability to read, write, speak, and understand English.
  • Ability to make independent decisions when circumstances warrant such action.
Preferred Qualifications
  • Associate’s or better in Nursing.
  • Bachelor’s or better in Nursing.
  • Technical or other training in Nursing.
  • Registered Nurse.
  • License Practical Nurse.
