Primary Care Provider - Geriatrics

Nation‑wide Primary Care organization geared towards the geriatric population is seeking a Primary Care Provider in CT. This is a full‑time permanent position with a 4‑to‑6 week orientation/training period, stock options, extensive PTO, a large CME stipend, malpractice coverage, and 401(k) with match available from day one.

• Monday‑Friday, 8 am‑5 pm schedule (no nights or weekends).
• 12‑16 patients per day once fully ramped up.
• Collaborative care team – 1 MD, APP, scribe, MA, shared RN/social work support.
• Specialty support from partnership with outside company.
• EPIC EMR.
• Call schedule rotated amongst markets; call center triage limits calls and allows calls from home.

• Board certification required.
• CT state license required.
• Training as a Family or Adult NP to be considered.
• Previous primary‑care or skilled‑nursing‑facility/nursing‑home experience preferred but not required.
• Bilingual Spanish preferred in some locations.
• 2+ years of experience in primary care.
