In-Home Health - Nurse Practitioner or Physician Assistant CT

Job Overview

Signify Health, part of CVS Health®, is hiring a full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. You will conduct 45‑minute visits in patients’ homes, performing medication/medical history reviews, physical exams, and when ordered, diagnostic tests such as diabetic eye exams, spirometry, etc. You will not prescribe medication, order lab tests, or alter treatment plans.

• Perform in‑home health evaluations and build trusted relationships with patients.
• Conduct medication and medical history reviews, physical evaluations, and coordinate diagnostic tests.
• Use iPad and clinical workflow tools for documentation.
• Travel locally and within licensed states as required.
• Conduct virtual visits when necessary.
• Provide diagnostic and preventive services including spirometry, DEE, bone density screening, etc.
• Adhere to safety protocols in diverse home environments, including those with pets or allergens.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Experience as a NP or PA.
• Ability to travel to New London and surrounding area.

• Experience with in‑home visits or telehealth.
• Knowledge of diagnostic and preventive service products.

Compensation ranges from $95,738 to $206,206 annually, based on experience, education, geography, and other factors.
