In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Bridgeport, CT

Role Overview

Signify Health, part of CVS Health, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations in the comfort of patients' homes.

• Conduct in‑home visits lasting about 45 minutes, including medication and medical history review, physical evaluation, and diagnostic tests as ordered.
• Use an iPad and clinical workflow; not required to prescribe medication, order lab tests, or alter current treatment regimens.
• Work flexibly to travel locally and within licensed states, and conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services products such as spirometry, diabetic eye exams, bone density screening, etc.
• Build trusted relationships, refer patients to appropriate care providers, and connect patients to the right care.

• Active, unrestricted NHS or PA license(s) in the coverage area(s).
• Board certification required; multi‑state licenses preferred.
• Ability to cover local primary locations such as Bridgeport and surrounding area.

Part‑time: 29.5 hours per week. Pay for each completed In‑Home Health Evaluation ranges from $46.03 to $99.14 per hour, depending on experience, education, geography, and other factors.
