REGISTERED NURSE - HOME CARE LIAISON

DAYS WITH WKD AND HOLIDAY ROTATION

Responsibilities
  • Screen and accurately complete referrals ensuring patients meet agency admission criteria and comply with state and federal regulations.
  • Process referrals for all programs (Traditional, Peds, and Infusion Therapy) from hospital sites and MD offices, determining patient qualification for home care based on established guidelines.
  • Verify home health care benefits per payor plans and obtain authorizations as indicated.
  • Obtain physicians’ initial home care orders.
  • Identify risk management situations and complete high‑risk referral processes on appropriate patients per guidelines.
  • Interpret company services, admission criteria, and medical policies to referral sources and patients seeking information.
  • Document initial plan of home health care and initiate interdisciplinary communication.
  • Interview patients/families in the hospital or SNF to assess and evaluate patient for home care appropriateness.
  • Collaborate with care‑management staff to assist the patient/family in initiating home care services and respond to inquiries.
  • Work within established productivity guidelines for assigned sites and report productivity daily to the supervisor.
  • Train patients/families on using equipment (e.g., pump) at home.
  • Provide quality customer service; promote collaboration and coordination between referral sources and home care services to promote continuity of care and effective use of resources.
  • Act as a resource to physicians, physician offices, patients, families, hospital and skilled nursing facility staff regarding types of services and criteria for home care, including Medicare guidelines.
  • Provide education to the care team on services that may enhance transitions of care including DME, Hospice, Palliative Care, Pediatrics, and Home Infusion Therapy.
  • Maintain knowledge of Medicare, Medicaid, and HMO procedures.
  • Attend site‑based multidisciplinary meetings (readmissions, capture, discharge planning) involving Advocate at Home patients.
  • Ensure follow‑up on patient satisfaction issues related to home care services.
  • Provide courteous service to patients, families, staff, and physicians.
  • Ensure complete referral information is obtained for accurate and timely billing.
  • Collaborate with insurance verification to achieve optimal reimbursement.
  • Assist in developing policies and procedures to ensure compliance with current practice standards.
  • Obtain Face‑to‑Face (F2F) encounter forms for all Medicare, Medicaid, and pending patients during referral process or within the first 7 days.
  • Educate physicians on the Face‑to‑Face process.
  • Maintain clinical skills and competency by attending monthly training and interdisciplinary team meetings.
  • Provide suggestions or improvements in current practices (process, documentation, etc.) to enhance quality, productivity, and customer satisfaction.
  • Exhibit a positive attitude and self‑management, contributing to a positive workplace culture.
  • Provide input on performance and compliance with standards, and adapt to changing business needs and responsibilities.
  • Partner with peers to achieve productivity levels and adhere to standard operating procedures during referral processing.
  • Participate in precepting new hire training and orientation.
  • Collaborate with other Advocate at Home product lines to provide comprehensive patient hand‑offs when appropriate.
Licensure

Registered Nurse (RN)

Education/Experience Required

RN with 3‑4 years of experience; 1‑2 years in home care preferred.

Knowledge, Skills & Abilities Required
  • Very good communication skills.
  • Knowledge of Medicare rules and regulations.
  • Experience in multi‑tasking on multiple computer systems.
  • Strong customer service skills.
  • Working knowledge of managed care environment.
  • Critical thinking skills.
  • Current RN licensure.
Physical Requirements and Working Conditions
  • On‑call and weekend rotation.
  • Ability to work in a fast‑paced environment.
  • Ability to drive to referral sites in all weather conditions and to corporate office for meetings and training/education.
  • When position involves direct patient care or contact, lifting up to 35 lbs without assistance; for over 35 lbs, use patient handling equipment with at least one other associate, when available. Unique patient lifting situations will be assessed case‑by‑case.
