Remote RN Care Coach — Chronic Care Management

Circle Link Health is seeking an Illinois registered nurse to work remotely as an RN Care Coach in a part-time capacity. The role involves managing and supporting patients with chronic conditions enrolled in Medicare’s Chronic Care Management Program. Responsibilities include planning and conducting monthly calls, maintaining patient records, and coordinating care. Ideal candidates should possess excellent documentation skills, strong time management, and a minimum of 3 years of nursing experience.
