Your Role on the Team
Position Summary:
Incumbent manages the care of the Advance Heart failure and Mechanical Circulatory Support (MCS) patient population across the continuum in collaboration with physicians and other members of the multidisciplinary team. Provides advanced assessment, planning and coordination of care, modification of the treatment plan and monitoring and evaluating outcomes. The Nurse Practitioner provides routine care and management of acute and chronic complaints.
The Nurse Practitioner also functions as an expert clinician, consultant, educator and leader in cardiovascular patient care management. Maintains a system for coordinating the caseload of care throughout episodes on mainly inpatient care, and occasional outpatient care.
- Valid CA Nurse Practitioner Furnishing Number and DEA Registration number
- Previous experience working as a Nurse Practitioner
- Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
- Must possess current BLS certification or attain prior to hire
- Must demonstrate customer service skills appropriate to the job
- Must be able to participate in on call rotation and work occasional weekends & evenings
- Minimum two (2) years of experience in acute care with VAD/MCS
- Minimum of two (2) years critical care experience
- Excellent written and verbal communication skills in English
- Current California RN license
- Current California Nurse Practitioner Licensure
- Ability to obtain UCIMC Allied Health Professional Privileges prior to hire
- Ability to maintain a work pace appropriate to the workload
- Ability to establish and maintain effective working relationships across the Health System
- ACLS within 30 days of employment
- Specialty nursing certification relating to the patient population
- Knowledge of University and medical center organizations, policies, procedures and forms
- Bilingual in English and Spanish
