SummaryThe LVN Home Health Nurse position requires a diverse set of skills to provide high‑quality care to patients in their homes. The ideal candidate will possess core skills such as catheterization, basic life support, electronic health record systems, patient care, and patient assessment. The LVN Home Health Nurse will be responsible for delivering compassionate care, utilizing their nursing expertise, and ensuring compliance with HIPAA regulations while providing services in a home health setting.
Responsibilities- Perform patient care functions & skilled nursing to patients in their home
- Coordinate care with registered nurses
- Provide accurate and timely documentation
- Assess and provide patient and family/caregiver education
- Compassionate and caring demeanor
- Currently licensed as an LVN in CA
- Valid CA Driver License
- Home health experience preferred but not required
