ResponsibilitiesAs a Per-diem
Case Manager (RN), you will oversee and manage the delivery of care for home health patients from pre-admission through post-admission. You will play a pivotal role in ensuring patients and their families receive appropriate care coordination and utilization of services to ensure the highest quality care.
- Managing care for Home Health patients throughout the care continuum, including pre-admission, admission, and post-admission stages.
- Coordinating services to ensure that patients needs are met in a timely and effective manner.
- Collaborating with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans based on patient needs.
- Monitoring the progress of patient and ensuring appropriate interventions are implemented when necessary.
- Facilitating smooth transitions of care, ensuring all services and resources are in place for each patient.
- Advocating for patients and families, ensuring their needs are met, and providing education as needed.
- Successfully graduated from an approved and certified RN program
- At least 1 year of clinical experience as an RN
- Current and unencumbered license to practice as an RN in the state
- Ability to make quick judgments and work independently
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
- 2 years of experience as an RN
- Active patient or bedside care experience within the past three years
