As a Per-diem
Case Manager (RN), you will oversee and manage the delivery of care for home health patients from pre-admission through post-admission. You will play a pivotal role in ensuring patients and their families receive appropriate care coordination and utilization of services to ensure the highest quality care.

• Managing care for Home Health patients throughout the care continuum, including pre-admission, admission, and post-admission stages.
• Coordinating services to ensure that patients needs are met in a timely and effective manner.
• Collaborating with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans based on patient needs.
• Monitoring the progress of patient and ensuring appropriate interventions are implemented when necessary.
• Facilitating smooth transitions of care, ensuring all services and resources are in place for each patient.
• Advocating for patients and families, ensuring their needs are met, and providing education as needed.

• Successfully graduated from an approved and certified RN program
• At least 1 year of clinical experience as an RN
• Current and unencumbered license to practice as an RN in the state
• Ability to make quick judgments and work independently
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care

• 2 years of experience as an RN
• Active patient or bedside care experience within the past three years

Compensation for roles varies depending on factors such as location, role, skill set, and level of experience. As required by state or local law, we provide a reasonable pay scale, including the hourly or salary range we reasonably expect to pay for this role.
