Home Hemodialysis – Nephrology Program
8-Hour shifts; days, evenings, weekends (subject to change)

DUTIES AND RESPONSIBILITIES
Use the nursing process in providing patient care.
Participate as a member of the Health Care Team and the Nursing Team.
Maintain competence and professionalism relevant to current practice.
Maintain nursing records.
Function as a member of a multidisciplinary team.
Train patients, family members, community agency staff and others.
Perform home hemodialysis in the home environment; manage all aspects of care for the dialysis patient; provide patient and family with advice, counselling and teaching related to the condition.
Be able to travel to complete home visits for patients.
Travel between Lakeridge Health sites as required.
Perform other related duties as assigned.
QUALIFICATIONS
Registered Practical Nurse diploma from an accredited community college.
Registration with the College of Nurses of Ontario (in good standing).
Current B.C.L.S.
A minimum of 2 years of recent Regional Nephrology Program experience (e.g., in-centre hemodialysis, home dialysis modalities; home hemodialysis therapies preferred including conventional, short daily and nocturnal).
Updated nursing skills including aseptic techniques and patient assessment courses.
Current certificate in medication administration.
Experience caring for Nephrology patients preferred.
Experience caring for Hemodialysis patients preferred.
Recent experience with equipment related to home therapies including the hemodialysis machines, R.O. and other water purification equipment, centrifuge, various water and blood detectors preferred.
Demonstrated aptitude for fostering client independence and training clients to manage self‑care dialysis at home; experience in teaching client/families preferred.
Demonstrated self‑motivation regarding continuing education in areas of patient assessment, patient education, leadership and related clinical specialties.
Sound knowledge and application of nursing skills as listed in the CNO "Standards of Nursing Practice".
Proven good organizational, interpersonal, problem‑solving and teaching skills.
Ability to prioritize and complete assignments in a reasonable length of time.
Demonstrated sound judgement, good observation and patient assessment skills, tact and empathy.
Demonstrated ability to perform appropriately in emergency situations.
Proven ability to perform a leadership/preceptorship role.
Evidence of continuing education related to the requirements of the position:
Nephrology certificate or evidence of progression towards completion.
Demonstrated commitment to maintain Reflective Practice Standard as outlined by the College of Nurses of Ontario.
Effective organizational, interpersonal, problem‑solving and teaching skills.
Ability to effectively prioritize work.
Proven communication skills to effectively interact with patients and their families, physicians and colleagues.
Physically capable of performing the duties of the position in order to lift, turn, and transfer patients manually and with mechanical aids.
Must have satisfactory performance record.
