Job Description
The Nurse Practitioner (NP) is a nurse who has an advanced level of clinical nursing practice that maximizes the use of graduate educational preparation, in-depth nursing knowledge and expertise in meeting the health needs of individuals, families, groups, committees and populations. The NP brings a holistic approach to care, addressing mental, physical and social aspects. Practices independently within the legislated scope of practice.
Responsibilities
Monitors ongoing conditions for patients with stable and chronic illnesses.
Provides family support, education and counseling on diagnosis, treatment options, evaluation outcomes and follow-up plans with a focus on health promotion, illness prevention and timely reintegration into the community.
Works with community partners in planning, treatment, discharge planning and community reintegration.
Monitors medical complications due to psychopharmacological drug use.
Supports the development of behavioral management interventions in collaboration with the psychiatrist/physician and the health care team.
In collaboration with the Primary Clinician, assists in development of the initial care plan, based on assessment of patient needs.
Provides consultation to the treatment, community agencies and other health care providers about difficult issues, ethical practice and managing complex care needs.
Integrates evidence-based research findings into clinical practice.
Participates in the dissemination and utilization of best practice guidelines.
Requirements
BScN
NP-PHC required
Current registration with College of Nurses in Ontario (CNO)
Excellent health assessment skills combined with sound clinical judgement and diagnostic skills relevant to dealing with a wide variety of health concerns complicated by mental health issues
Strong communication and interpersonal skills
Interest in exploring innovative service delivery approaches
Demonstrated ability to work in a multidisciplinary team environment with both medical and non‑medical personnel
Current CPR – Health Care Provider or commitment to obtain certification prior to start date
Preferred 2 years Mental Health or related experience
Minimum 2 years in a Nurse Practitioner role preferred
Knowledge of general and psychiatric nursing theory and practice, infection risk management, health promotion and health education/teaching activities
Experience with Meditech 6.0 is an asset
