We are seeking a dedicated and motivated Practice Nurse to join our friendly and enthusiastic team of Nurses and Healthcare Assistants. The successful applicant will play a key role in delivering high quality nursing care to our patients and supporting the ongoing development ofour clinical services.

Working part time, 2-3 days per week, you will providea wide range of Practice Nurse duties, including:

  • Chronic Disease Management (Diabetes, COPD, Asthma)
  • Experience of QOF and an understanding of achieving clinical indicators
  • Childhoodand Travel Immunisations
  • Cervical Screening
  • Dressingand Wound Care
  • Contraceptionservices
  • ECG recording
  • Health promotion, lifestyleadvice, and signposting to relevant services when appropriate

Main duties of the job

As a Practice Nurse, you will be responsible for delivering high quality general nursing services and chronic disease management to our patients. Working collaboratively within our multidisciplinary team, youwill help meet the needs of our population, support improved health outcomes,and contribute to maintaining the practices performance objectives.

Duties and

Responsibilities include:

  • Applying a comprehensive understanding of chronic disease management.
  • Monitoring and reviewing patients with chronic conditions and comorbidities.
  • Providing advice and support to promote effective self-management, following practice protocols and local formularies.
  • Contributing to the development of practice protocols, audits, and quality improvement activities.
  • Keeping clinical knowledge and skills up to date in accordance with current guidelines.
  • Delivering general practice nursing services including cervical screening, childhoodimmunisations, travel vaccinations, wound and ulcer care, and venepuncture.
  • Assisting and performing routine patient care tasks as directed by senior nurses and GPs.
  • Supporting a positive, collaborative, and healthy working environment.

Job Summary

The post holder is a caring, committed, and experienced registered nurse who,acting within their professional boundaries, will provide high quality care for patients from initial history taking through to clinical assessment, diagnosis,treatment, and evaluation. They will demonstrate safe and competent clinicaldecision making, delivering expert nursing care including assessment,diagnostic skills, and prescribing where applicable, within the general practice environment.

Job Responsibilities

Diagnosing and Treating Patients with Minor Illness

  • Demonstrate safe and competent clinical decision making, delivering expert nursing care and prescribing where applicable within the general practice environment.

Management of Chronic Disease

  • Contribute to chronic disease management through assessment, planning, monitoring, and reviewing patient progress.
  • Support the clinical team in identifying,recalling, and targeting patients for assessment and treatment.
  • Work within NHS and practice protocols todeliver effective long term condition management.

Providing Assessment, Treatment, Screening Services & Health Education

You will provide high quality nursing care across a broad range of services including:

Chronic Disease Management

  • Asthma
  • COPD
  • Diabetes
  • Health screening
  • Cervical screening (smears/cytology)
  • HRT
  • Contraception, Depo injections, pill checks
  • Removal of IUCD/IUS
  • Pessary care

Immunisations & Injections

  • Routine immunisations and vaccinations
  • Childhood immunisations
  • Travel vaccinations and followup
  • Zoladex implants

Treatment Room Duties

  • Wound care and dressings
  • Removal of sutures and clips
  • Minor injury/illness assessment
  • Assisting with minor procedures
  • Breath testsECGs
  • Blood pressure monitoring
  • Urinalysis

Health Promotion & General Practice Support

  • New patient registration checks
  • General lifestyle and dietary advice
  • Supporting patients with self management
  • Contributing to health education initiatives
Person Specification Qualifications
  • NMC registration required
  • Additional specific nursing skills / qualifications - ideally trained in at least one chronic disease management discipline: asthma / COPD /
  • diabetes
Experience
  • Experience of working within a nursing team
  • Experience of working within a GP Practice
