INTEGRAcare is looking for an enthusiastic Care Manager with a focus on client care and team leadership. In this role, you will coordinate and schedule primary care for clients, develop dynamic care plans, and manage a team of caregivers. Strong communication skills and a passion for nurturing quality care are essential for success.
Key Responsibilities:
• Manage nursing support and primary care for clients
• Develop and maintain client care plans and schedules
• Coach teams of caregivers and collaborate with multidisciplinary teams
• Teach new graduate PSWs the essentials of client care
• Participate in on-call rotation and team-building events
Requirements:
• Valid CNO registration in good standing
• 1–2 years of nursing experience preferred
• Bilingual in English and French
• Strong attention to detail and communication skills
• Valid driver’s license in good standing
