Key Responsibilities:

• Assess family needs and existing resources

• Provide case management and system navigation support

• Assist families in accessing healthcare and community services

• Coach families in completing funding applications

• Liaise with clinicians and funding agencies

Requirements:

• College diploma in Social Services or related field

• Minimum three years of experience supporting families

• Experience with children with neurodevelopmental needs

• Bilingualism in French and English required

• Valid driver’s license and access to a vehicle
