Coordinate referrals and intake for children's rehabilitation with a focus on family engagement and documentation. This part-time role requires skilled interpersonal communication and strong clinical judgment.
The Navigator, also known as Intake Worker, will facilitate the referral process and manage waitlists for youth in the rehabilitation program. You will use excellent assessment skills to ensure effective communication while providing support to families. Collaboration with a multidisciplinary team and adapting to changing demands are essential components of this role.
Key Responsibilities:
• Manage and triage referrals and intake
• Engage families with clear communication
• Oversee waitlist management for efficiency
• Maintain accurate documentation and data tracking
• Support quality improvement initiatives in collaboration
Requirements:
• Bachelor's degree in social work
• Good standing with a professional college in Ontario
• Experience in coordination and navigation
• Strong interpersonal and empathetic skills
• Ability to work effectively in multidisciplinary teams
