Home Care Nurse - RN - LPN

Bright Star Care is a home care agency that helps clients stay in their homes for as long as possible. We are looking for an RN or LPN to provide care for a 19‑year‑old male client with TBI, wheelchair‑bound, limited speech, and hemiplegic. The RN/LPN will work 12‑hour shifts, day and night, seven days a week.

• Park City:
  Every day of the week, 12 hour shifts days and nights, 7‑7

• Medication set up and oversight
• Provide thoughtful care to clients based on personalized Plan of Care
• Provide in‑home assessments
• Monitor and track changes in the client’s conditions and needs, and keep the Director of Nursing and any other appropriate personnel informed
• Collaborate with client’s family on plan of care, health changes, and concerns
• Weekdays include traveling to school with client
• Total personal cares

• Must have at least 1 year nursing experience
• Current unrestricted RN or LPN license in Utah
• Certified CPR Certification
• Negative TB skin test or chest x‑ray
• Must be able to pass a drug test and a background check
• Licensed driver with valid automobile insurance
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Must be able to lift 75 pounds
