Registered Nurse

Primary Purpose of Position

North Shore Community Health (NSCH) seeks a Registered Nurse to work under the Clinical Nurse Manager, providing direct and indirect nursing care, patient education, medication refill management, triage/assessment by phone and in person, and collaboration with the triage nurse team. The RN will support a safe and positive work environment by working with site supervisor, medical director, and community staff.

• Assess individual’s basic health status and related patient care needs for patients who call the clinic or who present for scheduled or unscheduled care.
• Analyze and interpret data and make informed judgments related to patient care.
• Evaluate outcomes of basic nursing interventions and initiate changes in care plans.
• Collaborate and communicate with primary care providers and other care team members to ensure quality and continuity of care, including participation in huddles to help ensure a smooth workday and maximize time with patients in the clinic.
• Review refill requests from pharmacy and patients based on medication refill protocol; reconcile medication lists and attach pertinent information necessary for the provider to authorize the refill.
• Administer immunizations and medications and perform other procedures as needed, including but not limited to EKGs, suture removals, wound dressing changes, ear irrigations, PPD application and interpretation, and blood pressure checks.
• Maintain accurate and timely documentation of all nursing care and patient communication, including phone notes, nursing visit notes, and immunization or medication administration documentation.
• Complete patient forms to the furthest extent possible within the scope of nursing practice, including prior authorization forms.
• Provide health education to individuals and families to further patient self‑management and self‑empowerment.
• Communicate with members of the patient’s care team outside the health center as needed, such as with specialists or visiting nurses.
• Manage vaccination supply and ordering, including proper vaccination storage and handling.
• Use information about data trends for self‑directed learning and performance improvement.
• Provide coverage at other NSCH clinical sites or call centers as needed.
• Perform additional duties to fulfill NSCH’s mission as assigned.

• College degree from an accredited school of nursing, current license to practice as an RN in the Commonwealth of Massachusetts.
• Current CPR certification.
• Ability to function effectively as a leader and a member of a team, and to work independently.

• Preference given to bilingual/bicultural individuals who can speak Spanish and/or Portuguese.
• Experience with electronic health records and strong computer skills.
• Sensitivity/skill in working with patients with complex medical, psychosocial, and socioeconomic circumstances.
• Ability to travel to other NSCH sites for meetings and periodic clinical coverage; some evening and Saturday hours required.
• Completion of NSCH‑required Compliance Training before the deadline.

All employees must receive required vaccinations and health screenings yearly, including the COVID‑19 vaccine. Proof of COVID‑19 vaccination must be provided prior to the first day of employment, unless a religious or medical exception is granted.

Hourly rate: $38.00 – $44.50 per hour.
