Seeking elite Licensed Practical Nurse! Previous home health experience preferred. This is a full‑time position with the expectation of 30 productivity points per week. The clinician will be paid per point and can earn additional pay by performing more than 30 productivity points per week.

• Comfortable treating wounds
• Serve as the patient’s advocate and maintain ongoing communication with physicians, referral sources, caregivers, and facility management to facilitate coordination of care
• Administer medications and treatments as prescribed by physician. Provide direct patient care including infusion therapy, wound care, teaching, and disease management
• Participate in regular case conferences and in‑services
• Service approximately 6 patients a day, Monday to Friday
• Competence to carry out any skilled procedure that the physician orders within the scope of the Nurse Practice Act

• Strong organization skills and time management
• Able to multitask and meet timelines
• Professional appearance and effective communication skills
• Current Texas LPN license
• Three years of clinical nursing experience
• Current Texas driver’s license
• CPR required
• Experience with healthcare software
