What You’ll DoPeterborough Regional Health Centre (PRHC) is pleased to partner with various Peterborough agencies, to offer services through the new Peterborough Homelessness Addiction and Recovery Team (HART) Hub. The Peterborough HART Hub is designed as a low‑barrier, trauma‑informed, culturally responsive, health care model that integrates primary care, mental health, substance use treatment, housing with supports, and health systems navigation. The Nurse Practitioner (NP) would be responsible for delivery services effectively and in a way that meets the needs of the individuals they serve.
The Registered Nurse, Extended Class, RN(EC) will work with clients presenting with significant mental health and/or substance use concerns using a client‑centred, evidence‑based service model based on standards of best practice. This position has NP responsibilities for awareness and potential coverage of NP roles in all mental health outpatient programs.
The position provides bio‑psycho‑social assessment of patients presenting with signs and symptoms of moderate to severe mental health and addictions conditions and offers psychotherapeutic and pharmaceutical interventions designed to ameliorate such conditions.
Jointly reporting to the PRHC Manager, Adult Outpatient Mental Health & Addiction Programs, and the Fourcast Addiction Services HART Hub Manager, and with professional accountability under the Chief Nursing Officer (CNO), the RN(EC) must be credentialed according to hospital bylaws. The NP will work onsite at the Peterborough HART Hub as part of the Integrated Health Team, through daily onsite work, the members of the Integrated Health Team will report to the HART Hub manager.
Through a Memorandum of Understanding (MOU) between PRHC and Fourcast, the NP will report daily to the HART Hub manager with shared oversight of the role between the PRHC and Fourcast managers. The PRHC and HART Hub managers each hold distinct but complementary areas of oversight, with shared responsibility for collaboration, performance outcomes and service quality.
