• Provide direct patient care to a diverse population of seniors, utilizing your knowledge and skills to assess, plan, and implement individualized care plans.
• Administer medications, treatments, and therapies as prescribed by physicians and other healthcare professionals.
• Collaborate with interdisciplinary teams to ensure comprehensive and coordinated care.
• Monitor and report patient status to nursing staff, physicians, and other healthcare professionals.
• Participate in quality improvement activities to enhance patient outcomes and satisfaction.

• Current LPN licensure in the state of New Jersey.
