Job Overview

As a Center Well Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients get back to the life they love.

• Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to the physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their documentation completion.

• Med Surg, ICU, ER, or acute experience
• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Valid driver’s license, auto insurance, and reliable transportation

• One year nursing experience
• Home Health experience

• $54.00 – $75.00 pay per visit/unit
• $84,900 – $116,800 per year base pay

• This role is patient‑facing and is part of Humana’s Tuberculosis (TB) screening program; selection requires TB screening.

• The role requires a valid state driver’s license and personal vehicle liability insurance per state minimum limits (e.g., $25,000 bodily injury per person, $25,000 bodily injury per event, $10,000 property damage, or higher).

Scheduled weekly hours: 1 (full‑time basis of 40 hours). Compensation may vary based on location and demonstrated skills.
