Job Overview

Signify Health, part of CVS Health®, seeks a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to conduct in‑home health evaluations, providing medication review, physical assessment, and optional diagnostic tests.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as needed.
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE).
• Not required to prescribe medicine, order lab tests, or alter current treatment regimens.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Ability to cover primary locations:
  Cambridge and surrounding area.

• Multi‑state licenses (preferred).

As an in‑home clinician, your primary workplace is the private residence of health plan members. You must be able to perform duties safely in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

The typical pay range for this role is $95,738.00 – $. The actual base salary offer will depend on factors such as experience, education, geography, and other relevant factors.
