• Provide nursing care in patients' homes within a home health setting
• Manage a caseload with self-scheduling of patient visits preferred
• On-call responsibilities required
• Maintain flexibility in work hours and assignments
• Provide, teach, supervise, and document nursing care for patients in the home setting
• Collaborate with healthcare providers, professionals, and community representatives to coordinate patient care
• Evaluate patient and family responses to nursing interventions and adjust care plans accordingly
• Apply knowledge of home care reimbursement and regulatory guidelines to care delivery
• Demonstrate critical thinking, emotional stability in emergencies, and the ability to act independently under supervision
• Reside more than 50 miles from the facility to be eligible for this position
• Schedule considerations: no more than 5 days of RTO; requests must be submitted prior to offer

• Active MA RN license required and must be in hand at time of consideration
• Current AHA BLS certification required
• Minimum one year of acute care nursing experience required; hospital-based experience preferred
• Home health nursing experience required
• Certified Home Health Nurse (CHHN) certification required with demonstrated proficiency in OASIS
• Infusion Therapy Certification preferred
• BSN highly preferred
• Valid driver's license and personal vehicle required; able to drive in inclement weather
• Auto insurance required
• One supervisory signed reference from within the past two years required
• Current Home Health RN skills checklist (within the past year) required for consideration

Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined. Posted job title:
