RN Case Manager – Plantation (Full Time)

OverviewWe are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will play a critical role in coordinating patient care, ensuring that patients receive the necessary medical attention throughout their treatment journey. This position requires a strong understanding of clinical practices and an ability to work collaboratively with various healthcare professionals in a fast‑paced environment.
Duties- Assess and evaluate patients’ needs for care and develop individualized care plans.
- Coordinate with multidisciplinary teams, including physicians, social workers, and therapists, to facilitate optimal patient outcomes.
- Manage discharge planning processes to ensure safe transitions from hospital to home or other care settings.
- Maintain accurate medical records and documentation for all patient interactions and care plans.
- Provide education and support to patients and their families regarding treatment options, resources, and follow‑up care.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Collaborate with managed care organizations to ensure compliance with insurance requirements and facilitate authorization processes.
- Address behavioral health needs as part of comprehensive patient assessments.
- Current Registered Nurse (RN) license in the state of practice.
- Experience working in a Level I or Level II trauma center or emergency room preferred.
- Knowledge of critical care practices and discharge planning processes.
- Familiarity with managed care systems and medical records management.
- Strong understanding of physiology and behavioral health principles.
- Excellent communication skills, both verbal and written, with the ability to work effectively in a team‑oriented environment.
- Ability to handle multiple priorities while maintaining attention to detail in a fast‑paced setting.
- Previous experience in case management is highly desirable.
