Position: Charge Nurse (RN)

Charge Nurse (RN) – Skilled Nursing

Community: Grand Plains Rehabilitation and Skilled Nursing

Community Type: Skilled Nursing

Location: Pratt, Kansas

Shifts Available: Days – 8 or 12 hours

Employment Type: Full-Time / Part-Time

Pay: Starting at $37.50–$40.50/hour

Responsibilities
  • Oversee nursing services, administration, and clinical supervision during assigned shifts.
  • Ensure compliance with nursing care standards, policies, and procedures.
  • Supervise and guide nursing staff, including LPNs, CNAs, and CMAs.
  • Complete and update resident assessments, care plans, and clinical documentation.
  • Communicate changes in resident condition to families, physicians, and nursing leadership.
  • Administer medications and treatments per physician orders and facility protocols.
  • Conduct routine rounds to ensure resident needs are being met.
  • Address, document, and report incidents, accidents, and concerns, including suspected abuse or neglect.
  • Ensure appropriate supplies, equipment, and medications are available and communicate needs to leadership.
  • Collaborate with interdisciplinary teams to promote quality care and resident safety.

Qualifications
  • Licensure / Certification: Current Registered Nurse (RN) license required.
  • Experience &

    Skills:

    • Strong clinical judgment and leadership skills.
    • Familiarity with state and federal nursing regulations.
    • Experience in skilled nursing, long‑term care, or senior living preferred.

Physical Demands
  • Lift up to 25 lbs. and assist with resident transfers as needed.
  • Push, pull, and maneuver up to 100 lbs., including resident mobility assistance for up to 350 lbs.
