Responsibilities
The incumbent provides care, treatment, and service to Home Health and Hospice patients and their families in their place of residence within their scope of practice. Functions as a member of the interdisciplinary team.

Qualifications

Experience:

Two (2) years LVN

Experience with in the last 5 years
Recent experience as an LVN in a Home Health or Hospice Agency preferred.
Licensure:
Currently licensed as a LVN in California
Current California driver's license, possession of automobile insurance & access to an operating motor vehicle.
Current CPR/BLS card approved through the American Heart Association.
