Schedule:
Exempt, min 40 hours per week | Monday – Friday | 8 am – 4:30 pm | Weekend coverage every 4 weeks.

The Nurse Practitioner (NP) is a vital member of the interdisciplinary team, providing high‑quality, participant‑centered care to older adults in the PACE program. The NP delivers primary and preventive healthcare, including assessments, diagnosis, and treatment of acute and chronic conditions, prescribing medications, and coordinating care with the team. This role supports the health, safety, and independence of participants through a holistic, geriatrics‑focused approach and requires a proactive, compassionate provider committed to serving the aging population.
Key Responsibilities- Leadership &
Collaboration:- Partner with the Director of Interdisciplinary Operations to support projects and optimize provider workflows.
- Collaborate with community partners and vendors to enhance care delivery.
- Support the interdisciplinary team (IDT) in coordinating complex care.
- Clinical Care:
- Conduct health assessments, physical exams, and screenings to monitor participants’ health.
- Assess and treat acute medical needs to prevent hospitalizations.
- Lead advanced care planning discussions and update advanced directives.
- Patient Education & Care Planning:
- Educate participants and caregivers on managing chronic illnesses and treatment plans.
- Contribute to care plans that promote independence and address medical, functional, and psychosocial needs.
- Team Participation:
- Engage in IDT meetings, offering insights to optimize participant care.
- On-Call and After-Hours Support:
- Provide rotating on-call and weekend support as needed to address urgent care needs.

Skills, Knowledge & Expertise- Education & Certification:
- Master’s degree in nursing (NP) from an accredited program.
- Current, unrestricted Nurse Practitioner license in Michigan, required.
- CPR certification or willingness to obtain upon hire.
- Experience:
- Minimum of 1 year as a Nurse Practitioner.
- Experience with frail or older adult populations, preferred.

- Knowledge of geriatric care and psychosocial determinants of health.
- Effective communication and collaboration within an interdisciplinary team.
