Job Title: Case Manager

Pay: $28.15/hour

Hours: Monday - Friday, 8:00 AM - 5:00 PM

As a Case Manager, you'll be pivotal in supporting individuals by coordinating care, building service plans, and advocating for the needs of your consumers. You'll work closely with team members to ensure each person receives personalized services that enhance their quality of life and promote independence.

• Managing assigned consumer caseloads and completing follow-up tasks
• Coordinating and updating service plans for consumers
• Handling heavy phone communication with consumers, families, and providers
• Conducting face-to-face meetings, either in-office or during home visits
• Maintaining accurate case notes and documentation
• Following program guidelines and state compliance requirements
• Collaborating with internal teams to help reduce service backlogs

• Required:
  
  Bachelor’s degree in social science or a related field OR 4 years of work or volunteer experience directly supporting consumers in social service settings
• Experience in nonprofits, care facilities, disability services, senior services, or similar environments preferred
• Strong communication, organization, and time management skills
• Ability to manage multiple priorities in a fast-paced environment
• Comfort working independently while maintaining professionalism and empathy
• Valid Driver’s License
