General Description:
Exercise Physiologist/ Preventionist will provide outreach services to additional military treatment facilities and review consults, schedules patients, and coordinates multispecialty education classes to diabetics and pre-diabetics.

Specific Tasks:
• Obtain and maintain Centers for Disease Control (CDC) recognition of the Endocrinology Diabetes Center of Excellence (EDCoE) Diabetes Prevention Program (DPP).
• Subject matter expert in regards to EDCoE’s DPP.
• Obtain expert/master trainer certification for DPP.
• Develop and present briefings to individuals, class participants, and groups as requested within area of expertise. Programs include, but not limited to: Diabetes Self-Management Education [DSME] and Outreach programs – Diabetes Champion Course, ECHO, webinars).
• Develops/updates/implements/executes/promotes current DPP at Surgical Center.
• Annual review/revise DPP content for both the participant and facilitator curriculum.
• Assist in development, updating, and execution of virtual delivery of DPP via VA Virtual Medical Center (VMC) or other platforms that are developed.
• Promote DPP throughout Military Treatment Facilities (MTFs) as able, which includes, but not limited to: developing and disseminating marketing tools and participating in health fairs.
• Document all patient interactions and maintains accurate and current notes, records, and patient medical records of all patients seen, as appropriate, and produce reports of evaluation and/or treatment as required.
• Organize and counsel fitness, health, and prevention classes as outlined by current CDC guidelines for diabetes prevention program for the 1 year Diabetes Prevention Program; CDC guidelines subject to change and must update and keep current. Conduct weekly phone/computer educational reinforcement (for first 12 weeks) along with, at a minimum, monthly in-class one hour presentations for one (1) full year.
• Provide Core (12 wk) and Post-Core (52 wk) lifestyle management education across a range of modalities in accordance with the diabetes prevention program.
• Administer physical fitness assessment for patients at risk of diabetes; may include muscular fitness testing, body fat assessment, and exercise prescriptions.
• Maintain all patient information in an Access database (or other database) to assess program effectiveness. Review the data to assess effectiveness of program while conducting continual process improvement.
• Participate in research projects within the EDCoE as pertains to DPP.
• Consult with the Primary Diabetes Care Provider on appropriate exercise preparation.
• Assist in planning and execution of all aspects of the Endocrinology Diabetes Center of Excellence in concert with Air Force Diabetes Program strategic plan, goals, and objectives.
Position Requirements
Bachelor,   1 to 2 Years work experience
• Bachelor's degree in a health-related field from an accredited institution is required.
• Shall have Continuing Medical Education (CME) / Continuing Education Units (CEU).

• Minimum of two (2) years' experience in a health-related field is required within the last three (3) years.
• Training to be a trainer/presenter for the Centers for Disease Control (CDC) recognized Diabetes Prevention Program (DPP) utilized within the Endocrinology Diabetes Center of Excellence (EDCoE) required. Expected to obtain master trainer certification for CDC recognized DPP utilized within the EDCoE within 2 years.

• Shall have and maintain a current Basic Life Support (BLS) certification.
